NURSE PRACTITIONER - PERMANENT FULL-TIME

JOB DESCRIPTION
The Kapuskasing and Area Family Health Team (KAFHT) is dedicated to providing high-quality, patient-centered primary care. As a Nurse Practitioner, you will be an integral part of our interdisciplinary team, working collaboratively with physicians and allied health professionals to deliver comprehensive care.

KEY RESPONSIBILITIES

• Conduct comprehensive assessments, diagnose conditions, and create personalized treatment plans.
• Prescribe medications and manage chronic diseases using evidence-based guidelines.
• Order, perform, and interpret diagnostic test.
• Provide patient education and counseling to promote health and prevent disease.
• Collaborate with physicians and other healthcare professionals to ensure coordinated care.
• Maintain accurate and timely documentation in patient records.
• Participate in program development and quality improvement initiatives.
• Other duties that are deemed appropriate and assigned by the executive Director of the KAFHT.

JOB REQUIREMENTS

• Master of Science in Nursing (MScN) - Primary Care Nurse Practitioner
• Current registration as a Nurse Practitionner in good standing with the College of Nurses of Ontario
• Strong clinical decision-making, communication, and critical thinking skills.
• Ability to work independently and as part of a multidisciplinary team.
• Bilingualism (English & French) is an asset
• Proficiency with EMR (PS Suite) and computer software such as Outlook, Word, and Excel.
• Entrepreneurial spirit directed to helping KAFHT in the delivery of programs.

SALARY
$64.35 hourly + comprehensive benefits package
