Position Summary
To perform duties which provide optimum individualized resident care, under the direction of the Registered Nurse. Acts as a liaison (team leader) between the RN and the care team. Required to perform duties in accordance with the Standards of Nursing Practice, College of Nurses of Ontario, and Long Term Care Legislation.
Qualifications as per Regulations July 2010
- A Registered Practical Nurse with a current certificate of competence from the College of Nurses of
Other Qualifications
Post RPN education in gerontology or related field, preferred
Medication administration certificate from a community college, as recognized by Ministry of Health
Experience in Long Term Care setting with demonstrated leadership skills
Excellent verbal and written communication in English, able to communicate in French and English preferred
Ability to work with peers in a team approach
Physically able to perform assigned duties
Ability to perform shift work over 24 hour period in a 7 day a week operation
Computer literacy preferred
Advanced foot care, wound care, GPA, PIECES, or U First Training preferred
Experience with RAI-MDS preferred

- Adhere to established general and departmental policies and procedures.
- Identify and report problems and recommend changes to improve quality of service.
- Protect own health and health of others by adopting safe work practices, reporting unsafe conditions immediately, and attending all relevant in-services regarding occupational health & safety. Follow all guidelines for employees and employers as legislated under the Ontario Health & Safety Act.
- Act as an ambassador on issues related to the Home outside of the Home.
Position Responsibilities:

Performs the responsibilities of the position in accordance with Bill 140 and all other employment laws and regulations associated with providing care in a Long Term Care facility.
- Ensures individualized resident care is provided in a manner reflective of the Home's Mission Statement and Values.
- Responsibility for the safe, accurate receiving, storing, administration and discontinuing of medications and treatments provided to residents, while maintaining complete, current and accurate applicable documentation.
- Assisting with regular assessments of resident needs through the development, update, and evaluation of resident care plans and restorative plans (including long and short term goals).
- Participating in all aspects of resident care, while continually ensuring the safety of residents.
- Assisting in maintaining an environment to meet the physical, emotional, spiritual, and psychosocial needs of the residents.
- Assessing resident need for recreational and rehabilitation programs, as appropriate, and encourages resident participation in same.
- Assisting with supervision and serving of meals, nourishments, dietary supplements and documenting associated data.
- As a team leader, functioning as a liaison between all members of the care team and the Registered Nurse, while promoting and encouraging all team members to respect the individuality of each resident and their independence.
- Establishing a good rapport with residents (families and significant others), offering support as required.
- Ensuring daily, the neatness of residents' rooms (dressers, clothes closets), utility room, dining rooms, lounges, dining areas and bathrooms.
- Obtains appropriate professional development to ensure skills and abilities in LTC are maintained and enhanced.
- Assists members of the interdisciplinary team to ensure the needs of residents are met.
- Participates in all aspects of Infection Prevention and Control by following established practices such as ensuring the labelling of personal care equipment, cleaning and disinfection of resident care equipment and restocking of infection control supplies.
- Participates in continuous quality improvement of the Home and associated care services by:
- Ongoing collection and documentation of CQI data
- Using supplies in a cost efficient manner as per product/policy guidelines
- Offers suggestions to registered staff, DOC, administrators regarding resource allocation
- Participating in continuing education, relevant to LTC, to ensure skills and abilities are maintained and enhanced.
- Services on committees related to resident care.
- Assists in the evaluation of nursing staff under their supervision.
- Maintains regular attendance at work.
- Performs other related duties, as required.
